Reimbursement process:

Step 1: Fully complete the LOLA Claim form + cut out the original UPC label from the product packaging + locate the original purchase receipt for the product.

Step 2: Mail your completed claim form, original UPC label and original product purchase receipt to the address in the LOLA Claim form.

Step 3: Receive your reimbursement check for ONE (1) LOLA product, with a purchase price up to $14.98 USD (excludes taxes), in the mail. For complete details see the step-by-step instructions and Terms and Conditions on the LOLA Claim form.
